Only a child's legal guardian may register a student.

Student's Birth Certificate
Student's Immunization Record
Proof of Residency Documentation (can
be one of the following:)
-voter registration card
-lease agreement
-mortgage statement
-utility statement
-bank statement
-rent receipt
